Style Integration issues and Status bar not showing.

Hi everyone, I install DW Question & Answer, it is a great plugin, but I dont understand well how to make the Style Integration, I already tried looking for the guidelines and for others users questions in this page, but it still giving errors and I dont know what exactly to do, the page where i installed it on my site is http://espacocinema.com.br/forum/ The status bar are not showing I think this is because there is no integration yet. Someone can help me with the integration ? My Theme page.php is the following bellow. Thanks for your help.
