Unintended Duplicate Answers and sometimes Question

For some reason, when users (or I) post answers on my site – the answer sometimes shows up 2 or more times. I am wondering what might cause this and how to fix it?

It has happened now with a question too. But this is the first time.

This problem is typical with answers.

In this case, you can send me username & password of your site for further checking.
Please use the Private answer option.
