Pending question category not saved

I have set the option “Question must be manually approved” which causes questions to be entered with status “pending”.

However, the question category is lost.
When an admin goes to “publish” the question, there is not “question category” selected.

How can I fix this issue?

I removed some code in Handle->insert_question that caused this issue I think.
