Missing texts in Contact form (JetPack) and aligning fields in DW Page

We had issues with the plugin Contact Form 7 and we’re trying the Contact Form of JetPack. However, when I placed the contact form in the contact section of our Home Page, there are missing texts before the word "required" (see screenshot) specifically, the texts "Name", "Email", "Comment" and "Send." How can I make those texts visible? Also, How can the fields for Name and Email align with the Comment field? Here’s the screenshot: http://prntscr.com/958r2z
