Ask questions page delete all styles

Hello! I have a problem with the asking questions page.
I have tried in all possible ways and do it as I do, on the page where I insert in shortcode [dwqa-submit-question-form] all styles are deleted: logos, or sidebar are not shown. With the Free version of the plugin worked perfectly for me, but with the pro version it stopped working.
The images you can include on the ask questions page are not even shown …
