How can I add a Name field to the Answer form?

I have DW Q&A setup so that anonymous users can post answers to questions (to avoid having user accounts). How can I add a name field to the answer form so that we know who has answered the question? I understand that some users might not enter anything, which is fine but for those who do want to show their name, how can I give them this option?

Unfortunately, our theme does not support this issue in the plugin.
